I am starting to build a form with an image as input Help

I have many users that send me cemetery pictures and they are never in the same format so I want to give my users the ability to fill in the blanks, add the image and send me the file with their input.
In the future, I want this same data to go to a Database but I want to do a lot of checking the data first. For now, getting the file and data emailed to me is my next step.

Google will find you a lot of free php scripts. I found a lot that would fit your requirement when looking for something else earlier today. If I had known about your post I would have saved the search.
