I still struggle a bit with forms but looking at the code for your page there is one thing that strikes me at once. You have created a separate form for each field, so the send button that people click on no longer associated with the Bravenet account referenced higher up the page.
There should be a single pair of form and /form tags with everything else in between. You can put tables or divs inside the form to structure it.
