They should have sent you an e-mail when you signed up for the poll at bravenet with the html code on it.
But you can also get the html copy/paste code by
l. Login to your bravenet account
2. Find the registered services area and you should have an icon with poll on it in that area.
3. Click on it
4. Find the utilities section and there is an icon that says copy/paste html or something like that click on it and there is your code
