I have uploaded a survey that I am wanting to use for my master's project, but I can't get it to send the results to my email. Does anyone have any suggestions as to what I have done wrong in the coding?

Major problems: you have several

and

tags buried in the code.

Your form must have only one

tag, properly coded (normally as specified by Bravenet or other provider), all the elements of the form, including at least a submit button, normally also a reset button, then a closing

tag.

The major problem with your form, after you clean up the code, is that it will only work for your users who have an email client properly configured. That's not everyone! Lots of people are using free email accounts from hotmail, yahoo, gmail, etc.

I think I see the problem. If you type in the URL in your browser, you can use just www.webformdesigner.com. The browser will add the rest.

If you do that here, the forum software will add
http://pub28.bravenet.com/forum/2385378114/show/ to the URL.

The correct address is:
