My email forms will not sent w/ Mozilla or IE only Safari (web address)

Can anyone tell me what I need to change/add/remove
to get these forms to send through IE and Mozilla?
Everything works fine for me in Safari.

Only thing I can see is that you have defined the usernum and cpv fields twice. Get rid of the second copy of these hidden fields under the submit button and see it that helps.
