Everything you said to do previously worked. Now, I was asked to make changes (required fields) and I think I messed something up on my apply now page. I decided to make the required fields page in a test page so I didn't mess with the page I already had. I think I may have made some changes before I decided to have a test page. Help. The info doesn't submit now.

I am looking at it, but it is going to take a while. You have the extra added problem of trying to use Javascript in the Website Wizard. It can be done, but because of an oddity in the Website Wizard, most Javascript requires some adjustment to work right. I probably won't be able to get to it till this evening.

You have a closing /form tag in the middle of the code for the form, before the submit button.
