Enqury Form

Real poor idea to try and post an entire web page. It'a usually easier to just provide a link to a test web page. But, I was able to extract your page from the forum posting.

If you don't have CGI access, and are not going to write your own email form processor, all you need it a host that provides form mail service. Few people actually write their own CGI form processors. Bravenet, and many other hosts, offer email forms service. You can try out the Bravenet service for free. The email forms service manager will provide you with the necessary

and tags to make it work. The free service allows 30 return fields, which fits with your form. It's really a pretty simple opereation.

However, no matter which service you use, you will have problems. With the possible exception of "radio" buttons, all tags must have unique names. Many of your tag names are duplicated. This is going to cause same named fields to override each other and you will not see all the data.

You have a bit of work to do, but your form can be made to work.
