The basic form bravenet gives you for free only has two options, but
can i just add another by copying and pasting/rewriting the name one?

You're correct, you can indeed add new "input" boxes. Just one thing: every "input" has to have a unique name, so don't forget to change that after you've copied them. Like:

First name:
Last name:

Looks like this:

First name:
Last name:
